Kathryn McGuinness, BSN, RN, CDP, CMC
Founder & Care Manager, Advanced Professional
Kathryn McGuinness is the founder of KMc Care Management and brings more than 20 years of experience guiding older adults and their families through complex care decisions with clarity, compassion, and discretion. She has been a dedicated care manager for over 15 years and is an Advanced Professional member of the Aging Life Care Association (ALCA), a distinction she has held for more than a decade, reflecting her commitment to the highest standards in the field.
Prior to founding KMc Care Management, Kathryn provided care management services at one of the largest and most respected firms in the country, where she developed deep clinical expertise and emerged as a trusted leader. A licensed registered nurse in Massachusetts, she has served as a staff and charge nurse at Faulkner Hospital in Boston and as a psychiatric nurse case manager in home health care.
Kathryn is a Certified Dementia Practitioner® and Certified Care Manager, combining clinical insight with highly personalized, client-centered guidance. She earned her BSN, summa cum laude, from UMass Boston.
A respected leader in the profession, Kathryn serves on the Board of Directors of ALCA New England and on its Executive Board as Treasurer. She previously served as Education Committee Chairperson and was honored with the 2022 Susan Shapiro Leadership Award. She is known for her unwavering advocacy and concierge-level approach, partnering closely with families to ensure each client’s needs, preferences, and dignity remain at the center of every decision.

Danielle Agyeman, MSW, LICSW, CMC
Care Manager, Advanced Professional
Danielle is a Licensed Independent Clinical Social Worker in Massachusetts with more than 25 years of experience supporting older adults and their families. She is an Advanced Professional member of the Aging Life Care Association and a Certified Care Manager (CMC), reflecting her commitment to the highest standards of care.
She earned her Bachelor of Social Work from Regis College and her Master’s Degree in Clinical Social Work, with a certification in Gerontology, from Boston College. Her passion for elder care began early while working as a certified nursing assistant in long-term care.
Danielle has held both clinical and leadership roles in skilled nursing facilities, including Social Worker, Director of Social Services, and Director of Admissions and Case Management. She also brings valuable experience as a hospice social worker, guiding families through complex and emotional transitions.
Known for her empathetic and practical approach, Danielle specializes in dementia care and has provided education and training to both professional staff and family caregivers. She is dedicated to helping families navigate care decisions with clarity while ensuring each client receives thoughtful, dignified, and personalized support.

Kathleen Burns, MSHCA, BSN, RN
Care Manager
Kathleen Burns brings more than 30 years of nursing experience supporting older adults and their families through the complexities of aging. Her extensive clinical background provides a strong foundation for addressing both the medical and emotional aspects of care.
A licensed registered nurse in Massachusetts, Kathleen has broad experience in geriatric care across a variety of settings. She specializes in care coordination, supporting transitions, and helping families navigate decisions with clarity and confidence.
Kathleen holds a Master of Science in Healthcare Administration and Management and a post-master’s certificate in Quality and Safety from Southern New Hampshire University, as well as a Bachelor of Science in Nursing from Curry College. Her academic training informs her focus on patient safety, quality outcomes, and well-coordinated care.
She is known for her calm, thoughtful approach and her commitment to ensuring care is responsive, personalized, and aligned with each client’s needs and preferences.

Kristen Jordan, BS
Care Manager Assistant
Kristen Jordan brings more than a decade of hands-on experience supporting older adults in home settings, along with a warm, person-centered approach to care. She is dedicated to helping clients maintain dignity, independence, and quality of life while building meaningful, trusting relationships.
Kristen has a natural ability to create engaging and personalized experiences, from thoughtfully planned outings to day-to-day support tailored to each individual. Known for her outgoing personality and sense of humor, she fosters genuine connection and comfort with those she serves.
In addition to her caregiving experience, Kristen has a strong background in coordination, advocacy, and management. She has worked in healthcare and organizational settings, including Brigham and Women’s Hospital and Boston Children’s Hospital, and has experience managing complex logistics and family systems.
Kristen holds a Bachelor of Science in Biology from Trinity College and is passionate about supporting clients in living fulfilling lives at home.
